Things most newcomers don’t know
New York has a coverage tier between Medicaid and the marketplace that most states do not.
The Essential Plan is a Basic Health Program — an option in the Affordable Care Act that only a couple of states ever took up. It covers households above the Medicaid income line with no monthly premium and low cost sharing, exactly the band where someone in most states would be buying a subsidised commercial plan with a four-figure deductible. Newcomers on part-year or entry-level incomes routinely buy a plan they did not need because nothing in the national explainers mentions it.

The academic medical centre is in the city, which is not typical of a metro this size.
Albany Medical Center is a teaching hospital, a medical college and the region's only level I trauma centre, ten minutes from downtown. For a metro of about 900,000 that is unusual — most comparable American cities send complex cases to a larger one two hours away. If anyone in the household has a condition that needs specialist follow-up, this is a genuine argument for the region.

Medicaid here has no waiting period and no enrolment window.
New York expanded Medicaid and accepts applications year-round with no residency waiting period. That is not the American default — the non-expansion states further south leave a gap below the subsidy floor entirely. If your income might qualify, apply the week you arrive rather than waiting for an annual window that does not apply to you.
