The Lei das Finanças Regionais lets an autonomous region tax up to 30% below the national rates, and Madeira applies that differential at the maximum — extended for 2026 to cover brackets up to the ninth. IVA has its own regional schedule too: 22% standard, 12% intermediate and 4% reduced, the reduced rate having been cut from 5% on 1 October 2024. None of this is a special expat scheme; it applies to anyone tax-resident in the region. It is also the part of Madeira's tax position that most guides skip entirely while breathlessly describing the free zone, which does not apply to individuals at all.

The MIBC — the Zona Franca da Madeira — grants a 5% corporate income tax rate on eligible income to companies licensed there, under a state aid authorisation from the European Commission. The conditions are real: jobs actually based in Madeira, activity genuinely carried on from the island, minimum investment in some cases, and ceilings on the amount of income that can benefit, scaled to headcount. Entities licensed between 1 January 2015 and 31 December 2026 can hold the rate through to 31 December 2033. What it is not is a personal tax break. If you incorporate there and pay yourself a salary, that salary is taxed under Madeira's IRS like anyone else's. Anybody selling you the free zone as a way to lower your own income tax is describing something that does not exist.

Madeira runs its own health service — the Serviço de Saúde da Região Autónoma da Madeira, under the regional government, with 47 health centres across seven zones and one main hospital, the Dr. Nélio Mendonça in Funchal. You register at your centro de saúde as you would anywhere in Portugal, but the organisation, the appointment systems and the waiting lists are regional. SNS24 on 808 24 24 24 still works. What is genuinely different is that the island has one hospital: anything highly specialised means a flight to Lisbon or Porto, which SESARAM arranges and funds for referred patients but which is a real logistical fact of life for anyone with a complex condition.

Digital Nomads Madeira launched at Ponta do Sol in February 2021, run by Startup Madeira with the regional government, and it has since spread to Funchal, Machico, Santa Cruz, Jardim do Mar and Porto Santo. What it provides is free coworking space, a Slack community, an events calendar and help finding accommodation. It confers no immigration status whatsoever. Non-EU nationals still need a D8 visa applied for at a consulate before travelling, with income of roughly four times the Portuguese minimum wage — €3,680 a month on the 2026 minimum wage of €920, and reindexed each January. The programme's excellent press has left a lot of people believing Madeira issues something it does not.

Madeira is roughly 1,000 km from Lisbon in the Atlantic. Groceries, furniture, cars and building materials all come by ship or plane, which lifts prices on goods well above the mainland even though rent and services are cheaper. Mainland and European online retailers frequently exclude Madeira from free delivery, add substantial surcharges, or decline to ship bulky items at all. Budget for a step change in the cost of stuff rather than the cost of living generally, and check delivery terms before ordering anything large.

Madeira Airport's runway is famously extended on concrete columns over the sea and sits in a spot where crosswinds and turbulence regularly force diversions to Porto Santo, Tenerife or Lisbon, or outright cancellations. This is normal and not a sign of anything going wrong. If you are flying for a job interview, a wedding or a connecting long-haul, come a day early.
