List with a vårdcentral as soon as you are registered
You choose your primary care centre and list with it — a free choice across the region, done online with BankID or on paper. Närhälsan is the region's own public primary care provider and there are private ones inside the same system. Doing this before you are ill means you have a named point of contact rather than starting from the emergency department.

Call 1177 before going anywhere
1177 is answered by registered nurses around the clock. They assess, advise and direct you to the right level of care. Turning up at Sahlgrenska's emergency department without doing this is the classic newcomer mistake and produces very long waits — and it costs SEK 400 rather than SEK 200.

Know the actual Västra Götaland fees before you need them
A doctor's appointment at a public vårdcentral is SEK 200 and a nurse or psychologist appointment SEK 50; a private centre inside the system charges SEK 300 and SEK 100 respectively. A specialist is SEK 300, or SEK 100 for a first visit on referral. The emergency department is SEK 400 and an inpatient day SEK 130. A home visit adds SEK 100.

Track the high-cost ceiling and claim your frikort
Once your patient fees reach SEK 1,450 within a twelve-month period, a frikort applies and further outpatient care is free for the rest of that period — and it is valid across the whole of Sweden, not only in Västra Götaland. It is tracked digitally, but check yours rather than assuming, and note that vaccinations and certificates sit outside the ceiling entirely.

Register children and use the free child healthcare
Healthcare and dental care are free for everyone under 20 in Västra Götaland, and BVC child health centres provide structured developmental checks from birth. This is one of the strongest parts of the system and it is opt-in by registration.

Get insurance cover for the registration gap
Between arriving and being entered in the population register you are not yet in the system. EU/EEA arrivals use the EHIC; everyone else needs private cover for that window, which can run several weeks.

Things most newcomers don’t know

Healthcare is the region's business, so a colleague's advice from Stockholm about what a visit costs is simply wrong here.

Västra Götalandsregionen sets its own patient fees, decides how out-of-hours care is organised and runs its own choice-of-provider system. What is national is the 1177 number, the pharmaceutical ceiling, the care guarantee and the statutory maximum on the high-cost ceiling — which is why the SEK 1,450 frikort figure is the same everywhere while the SEK 200, SEK 300 and SEK 400 you pay to reach it are not. When someone quotes you a figure, ask which region they were in.

Choosing 1177 over the emergency department saves you both an afternoon and SEK 200.

A doctor's appointment at a public vårdcentral is SEK 200; the emergency department is SEK 400 and you are triaged behind genuine emergencies. Calling 1177 first puts a registered nurse between you and the system and they can direct you into the right service. The financial gap is small and the time gap can be most of a day.

The frikort is SEK 1,450 and the things people assume are inside it are not.

Once your outpatient patient fees reach SEK 1,450 in a rolling twelve months, further outpatient care is free. But vaccinations and medical certificates are explicitly outside the ceiling, and prescription medicines run on an entirely separate national high-cost scale. People count travel jabs towards the frikort and are surprised when it does not arrive.

The care guarantee gives you a legal right to go elsewhere when the wait is too long.

Sweden's national care guarantee sets maximum waiting times for contact, assessment and treatment. If your region cannot meet them it must offer care with another provider — including in another region — at no extra cost to you. Regions do not volunteer this and the onus is on the patient to ask. It is the most valuable and least used right in the Swedish system.

Common mistakes to avoid

Assuming Stockholm's patient fees apply — Västra Götaland sets its own.
Going to Sahlgrenska's emergency department instead of calling 1177 first.
Counting vaccinations and certificates towards the SEK 1,450 ceiling, which excludes them.
Assuming adult dental care is covered the way medical care is — it is a separate, far less generous scheme.
Arriving without cover for the weeks between landing and being registered.
