Establish what covers you — OSMS or private
Kazakhstan's compulsory social health insurance (OSMS) is funded by employer and employee contributions and administered against your IIN. If you are on a local payroll you will normally be inside it. If you are on a Neo Nomad visa or a visa-free stay you are not, and you need private insurance — which the nomad visas require anyway. Check your status against your IIN rather than assuming; contribution gaps suspend cover.

Register with a private clinic that has English-speaking staff
Almaty's private sector is the country's best: the international-standard clinics in the central and Bostandyk districts handle most expatriate care, with same-day appointments normal. A GP consultation typically runs KZT 10,000-20,000 and a specialist somewhat more — often less than a co-pay in Western Europe or the US. For anything serious, the private hospitals here are where people from elsewhere in Kazakhstan are referred.

Take the winter air seriously
From roughly October to April a temperature inversion traps vehicle exhaust and coal-heating smoke in the Almaty basin. PM2.5 regularly runs several times WHO guideline levels and the city has appeared in global top-ten most-polluted rankings on winter mornings. Practical steps: choose a flat in the higher southern districts where you can, buy an HEPA purifier for the bedroom, check the AQI before outdoor exercise, and move runs to the Alatau trails above the inversion layer.

Know the pharmacy system and the emergency number
Pharmacies (аптека) are dense and many are 24-hour; prescription rules are looser than in Western Europe and pharmacists advise and dispense a wide range directly. Carry the ACTIVE INGREDIENT name of anything you take regularly, because brand names differ across the region. The unified emergency number is 112; 101 fire, 102 police and 103 ambulance all still work and calls are free from any phone.

Things most newcomers don’t know

Almaty's winter smog is the single biggest health variable in choosing where to live in this city, and it maps onto altitude.

The inversion layer sits over the lower, northern part of the basin. Districts higher up the slope toward Medeu often sit above it on the worst mornings, which is why rents there carry a premium locals understand and newcomers read as merely 'nicer'. If anyone in the household has asthma, this is a housing decision, not an air-purifier decision.

OSMS coverage is tied to your IIN and to contribution history, so a gap in employment silently suspends your cover.

The system checks contributions rather than status. People who change jobs, go freelance for a few months, or whose employer falls behind on payments discover at the clinic desk that they are uninsured. Check your contribution status through the state portal periodically rather than at the point of need.

The nomad visas require medical insurance as an application document, which means you already have private cover — but check that it is valid for the whole visa period, not just the entry.

Travel policies bought to satisfy a visa checklist often run 90 days. The visa runs a year. The gap between the two is where people find themselves paying for a hospital admission out of pocket. Match the policy term to the visa term at the outset.

Dental and elective procedures in Almaty are a fraction of Western prices and the private clinics are used to foreign patients.

Low labour costs and a competitive private market make dentistry, imaging and minor procedures cheap enough that many foreign residents pay out of pocket and keep insurance only for hospitalisation — the same rational strategy that works in Tbilisi.
