Establish what covers you — OSMS or private
Compulsory social health insurance is funded by employer and employee contributions and administered against your IIN. On a local payroll you are normally inside it. On a Neo Nomad visa or a visa-free stay you are not, and you need private cover — which the nomad visas require as an application document. AIFC employees should check what their employer's package actually includes, since AIFC payroll arrangements differ from ordinary Kazakh employment. Verify your status against your IIN rather than assuming; contribution gaps suspend cover.

Register with a modern private clinic
Astana's hospital stock was largely built alongside the capital and is modern, with the national medical centres here. Private clinics handle most foreign residents with same-day appointments; a GP consultation typically runs KZT 10,000-20,000. English-speaking care is easier to find here than the city's size would suggest, because of the diplomatic and AIFC populations.

Take the cold seriously — this is the local health risk
January averages around -14°C and cold snaps reach -40°C, with wind on open ground driving the effective temperature lower. Frostbite on exposed skin happens in minutes at those temperatures. Buy Arctic-grade outerwear locally rather than assuming a European winter coat will do, learn the local habit of covering every centimetre of skin, and treat schools' and employers' cold-weather closure announcements as real safety measures rather than inconveniences. Vitamin D deficiency over a long dark winter is also a genuine and easily-tested issue.

Know the pharmacies and the emergency number
Pharmacies (аптека) are dense and many are 24-hour; prescription rules are looser than in Western Europe and pharmacists advise and dispense directly. Carry the ACTIVE INGREDIENT name of anything you take regularly, since brand names differ across the region. The unified emergency number is 112; 101 fire, 102 police and 103 ambulance all still work, and calls are free from any phone.

Things most newcomers don’t know

Cold injury is the realistic health risk here, and it is a matter of minutes rather than hours at -40°C.

Arrivals from temperate climates consistently underestimate this. Frostbite on exposed skin at those temperatures with wind is fast, and the danger is greatest in ordinary situations — waiting for a bus, walking between buildings — rather than in anything adventurous. Local practice is to cover everything, keep journeys between heated spaces short, and ride-hail distances you would walk anywhere else.

OSMS coverage tracks contributions against your IIN, so a gap in employment silently suspends your cover.

The system checks contribution history rather than status. People changing jobs, going freelance for a few months, or whose employer falls behind on payments find out at the clinic desk. Check your contribution status through the state portal periodically rather than at the point of need.

AIFC employment arrangements can sit outside ordinary Kazakh payroll, so do not assume OSMS enrolment.

The AIFC's own labour regime and its tax exemption for foreign employees mean that AIFC employment does not necessarily look like ordinary Kazakh employment on the contributions side. Ask HR specifically whether you are enrolled in OSMS and what private cover the package includes, rather than assuming a Kazakh employer implies the Kazakh system.

Astana's medical infrastructure is newer than Almaty's, but Almaty still has more depth in some specialties.

The capital's hospitals were built alongside the city and the national medical centres are here, so facilities are modern. Almaty, being older and larger, retains deeper specialist and academic medicine in some fields. For most needs this is irrelevant; for a specific serious condition it is worth asking where the country's centre of expertise for it actually is before assuming the capital.
