    Guayaquil has three parallel networks. The IESS runs the Hospital Teodoro Maldonado Carbo, the largest in its national system. The Ministerio de Salud Pública runs the free public hospitals. And the Junta de Beneficencia de Guayaquil, a private charitable body founded in 1888, runs several of the biggest — including the Luis Vernaza general hospital, the Roberto Gilbert children's hospital and the city's main maternity.     Treat the tap as non-potable and the heat as a real load

    Tap water is not drinkable — bottled or a proper filter, without exception. And a 26.4°C annual mean with high humidity is a genuine physiological load on someone arriving from a temperate country: expect a fortnight of poor sleep and low appetite, drink far more than feels necessary, and do not schedule anything demanding for your first days.

Things most newcomers don’t know

Guayaquil has a hospital system that exists nowhere else in Ecuador, and it is not the public one.

The Junta de Beneficencia de Guayaquil is a private charitable institution founded in 1888 that runs several of the city's largest and oldest hospitals, including the Luis Vernaza. It is neither a ministry hospital nor a commercial private one, it long predates the modern Ecuadorian state's health system, and it is a large part of why the city's hospital capacity looks the way it does. Newcomers who mentally sort every hospital into 'public' or 'private' get this wrong and then get the referral pathway wrong with it.

Swap altitude for the mosquito: the sierra's health problem and the coast's are opposites.

Quito's health advice is about 2,850 metres, acclimatisation and extreme ultraviolet exposure. None of that applies here. What applies instead is that Aedes aegypti thrives at sea level in this climate, so dengue is endemic rather than seasonal-only, with a hard peak in the January-to-April rains and chikungunya and Zika in the same vector. Anyone moving between the two cities carries the wrong precautions in both directions.
