Umowa o pracę enrols you automatically. Umowa o dzieło generally does not enrol you at all. B2B means you register and pay yourself, and the health contribution differs between the progressive scale, the flat rate and ryczałt.

GIOŚ publishes live particulate readings and the city issues alerts. On a windless January day the difference between districts is large enough to change whether you run outdoors or send a child to the park. Treat it as weather information, not activism.

Learn the out-of-hours route now
112 for emergencies, SOR departments for serious cases, and nocna i świąteczna opieka zdrowotna for ordinary problems outside 08:00–18:00 and at weekends. The last one is a separate service from your daytime clinic and knowing where yours is saves a wasted trip.

Arrange voluntary NFZ insurance if nothing else covers you
If your contract type leaves you outside the system you can contract voluntarily with NFZ, which also satisfies the insurance requirement on your residence application.

Things most newcomers don’t know

Winter air here is a health variable, not a talking point.

Silesian smog comes overwhelmingly from low-stack household emissions — coal and wood burned in old boilers in single-family houses — which is why it peaks on cold, still days and why it is worse in the low-rise fringes than beside the motorway. The voivodeship's anti-smog resolution has been retiring those boilers in stages since 2017, and it is genuinely improving. But if anyone in the household has asthma or COPD, the heating type of your street matters more than the postcode.

Insurance is checked electronically against your PESEL and nothing else.

The eWUŚ system returns a live green or red against your number when you present at a clinic. There is no card to show and no argument to make at the desk. An upstream administrative failure — a late employer filing, a dependant never registered — presents to you as simply being uninsured, with the fix somewhere else entirely.

Your spouse and children need a separate ZUS form, and it is usually forgotten.

Family members are covered only after the employer files a ZCNA registration for each of them. Nothing prompts it and nothing confirms it, and the omission surfaces when a child is turned away at a paediatric clinic. Asking HR to confirm the filing takes one email.

Silesia's specialist hospitals are a genuine regional advantage.

The conurbation carries a hospital density built for a much larger industrial workforce, including the cardiac centre in Zabrze that performed Poland's first heart transplant and still functions as a national reference unit. For serious specialist care this is one of the better places in Poland to live — the weakness is the ordinary outpatient queue, not the top of the system.

Common mistakes to avoid

Taking a B2B contract without working out who pays the health contribution and at what rate.
Assuming dependants are covered without the ZUS ZCNA filing.
Renting in a low-rise district heated by individual coal boilers without knowing that is what you are doing.
Never declaring a primary care clinic and turning up expecting to be registered on the spot.
Letting cover lapse in the gap between two jobs.
