NFZ, private packages, and a city with a winter air problem

An employment contract enrols you in NFZ automatically through your employer's ZUS contributions, and there is nothing to apply for. A B2B contract does not: you pay the health contribution yourself as part of your own ZUS. Public primary and emergency care is solid and free at the point of use; specialist queues are long, which is why nearly every professional employer here also provides a private package. On top of all that, Kraków has an air quality season.

Things most newcomers don’t know

The clinic desk checks your PESEL, and nothing else.

Poland runs eWUŚ, a live electronic check of insurance status. You give your PESEL, the system returns green or red, and that is the whole verification. It also means an administrative failure upstream — an employer that filed late, a family member never registered — presents as you being uninsured at the counter with no way to argue it there.

Your spouse and children are not covered just because you are.

Family members must be separately reported to ZUS on a ZCNA form, normally by your employer. It is a two-minute administrative act that is routinely forgotten during onboarding, and the failure only becomes visible when a child is refused at a paediatric clinic. Ask HR to confirm it has been filed rather than assuming.

Kraków banned solid fuel in 2019; the communes around it did not.

Since 1 September 2019 burning coal or wood inside the city has been unlawful, which measurably improved the air. But Kraków sits in a river basin that traps cold air, and the surrounding Małopolska municipalities are still replacing hundreds of thousands of old boilers. On a still January night the pollution simply drifts in. This is why the improvement inside the city has not ended the problem.

A private package is a normal benefit here, and the brand tells you almost nothing.

Nearly every professional employer in Kraków offers LuxMed, Medicover, Enel-Med or Scanmed, so an offer without one has a real gap worth negotiating. But packages are sold in tiers: which clinics, which specialists, whether diagnostics and dentistry are included. Two colleagues on the same-named provider can have very different access, and the detail is in the tier, not the logo.
