Health🇵🇱 Łódź, Poland

NFZ cover, the family form nobody files, and a city that is getting older

An employment contract enrols you in NFZ automatically through employer ZUS contributions. A B2B contract does not: you pay the health contribution yourself, at a rate that depends on which tax regime you elected. Łódź has a large medical university and its teaching hospitals, so specialist capacity is real — but it also has the oldest and fastest-shrinking population of Poland's big cities, which loads the system with exactly the demand that lengthens outpatient queues.

Total cost
Free at the point of use for the insured. On employment the contribution is deducted from gross pay. On ryczałt it is a fixed monthly amount stepping up with revenue band, half of it deductible. Employer private packages are close to standard in the outsourcing sector and can often be extended to family at cost.
Time needed
Cover starts with the contract. A clinic declaration is a same-day form. A public specialist appointment can be months away, which is what the private package is for.
Validity
Cover runs while contributions are paid, with a 30-day grace period after employment ends.

Medium confidence·Everyone living in Łódź. Cover follows your social insurance status, which follows your contract type — so the healthcare decision is made when you sign an offer, not when you need a doctor.

Before you start

  • PESEL
  • ZUS registration through an employer, your own business, or a voluntary NFZ agreement
  • A primary care clinic declaration
  • Proof of insurance, which your residence application also requires

Step-by-step

  1. 1

Confirm what your contract does to your insurance

Umowa o pracę enrols you automatically. Umowa o dzieło generally does not enrol you at all. B2B means you register and pay yourself, and the health contribution differs between the progressive scale, the flat rate and ryczałt.

  2. 2

    Declare a primary care clinic

Any NFZ przychodnia will take a deklaracja wyboru naming your GP. One form, free, changeable a limited number of times a year without charge.

  3. 3

Get your family registered on ZUS ZCNA

A non-working spouse and children are covered only once separately reported by your employer. It is a routine form and it is routinely forgotten during onboarding.

  4. 4

Read your private package tier, not its brand

LuxMed, Medicover and Enel-Med all operate in Łódź, and the outsourcing employers hand these out as standard. The networks here are smaller than in Warsaw — check which locations and specialists your specific tier includes before you rely on it.

  5. 5

    Learn the out-of-hours route now

112 for emergencies, SOR departments for serious cases, and nocna i świąteczna opieka zdrowotna for ordinary problems outside 08:00–18:00 and at weekends. The last one is a separate service from your daytime clinic and knowing where yours is saves a wasted trip.

  6. 6

Arrange voluntary NFZ insurance if nothing else covers you

If your contract type leaves you outside the system you can contract voluntarily with NFZ, which also satisfies the insurance requirement on your residence application.

Documents you’ll need

  • PESEL — the only thing a clinic desk actually checks
  • Proof of ZUS insurance or a voluntary NFZ agreement
  • Residence card or passport
  • ZUS ZCNA confirmation for each dependant
  • Private package card, where applicable

Things most newcomers don’t know

Demography is the local health story, and it cuts both ways.

Łódź has been losing population faster than any other large Polish city and the people who left were disproportionately young. That leaves an unusually old resident population served by a system sized for a bigger, younger city — good hospital and teaching capacity, but heavy demand on geriatrics, cardiology and oncology, which is where outpatient waits bite. For a newcomer in their thirties this mostly shows up as a longer specialist queue than the hospital count would suggest.

Insurance is checked electronically against your PESEL and nothing else.

The eWUŚ system returns a live green or red against your number when you present at a clinic. There is no card to show and no argument to make at the desk. An upstream administrative failure — a late employer filing, a dependant never registered — presents to you as simply being uninsured, with the fix somewhere else entirely.

Your spouse and children need a separate ZUS form, and it is usually forgotten.

Family members are covered only after the employer files a ZCNA registration for each of them. Nothing prompts it and nothing confirms it, and the omission surfaces when a child is turned away at a paediatric clinic. Asking HR to confirm the filing takes one email.

Your health contribution depends on the tax regime you chose, which surprises B2B contractors.

Since the 2022 reforms the health contribution for self-employed people is income-based rather than flat, and it is calculated differently under the progressive scale, the 19% flat rate and ryczałt. Under ryczałt it steps up in revenue bands and half of it is deductible. People compare income tax rates between regimes and forget that the health contribution can move the answer.

Common mistakes to avoid

  • Taking a B2B contract without working out who pays the health contribution and at what rate.
  • Assuming dependants are covered without the ZUS ZCNA filing.
  • Judging a private package by brand rather than by its Łódź clinic list.
  • Never declaring a primary care clinic and turning up expecting to be registered on the spot.
  • Letting cover lapse in the gap between two jobs.
