Check your licence qualifies for direct issue
The exchange applies to licences from countries party to the Convention on Road Traffic, and to qualifying mainland Chinese licences following the mutual-recognition arrangement. Learner and provisional licences are excluded, as are categories you obtained without an examination. A damaged, expired, lost or stolen licence cannot be exchanged.

Get the medical certificate and photos
A driver's health certificate is required. Any Macau clinic can issue one. Bring two recent colour photographs to the same size specification as an identity document.

Apply at the Transport Bureau (DSAT)
Submit the application in person or through a legal representative holding a properly authenticated power of attorney. Bring the resident ID card, the original foreign licence, the medical certificate, the photographs and any required supplementary documents — several jurisdictions, including Hong Kong, Taiwan, Japan, Australia, Malaysia, Korea, the Philippines and South Africa, need extra paperwork.

Reset your instincts for driving on the left
Macau follows Hong Kong, not the mainland. Roundabouts, overtaking and pedestrian sightlines all reverse the moment you cross the Hong Kong–Zhuhai–Macau bridge from Zhuhai. Give yourself a couple of low-traffic runs on Taipa and Coloane before attempting the peninsula.

Consider not driving at all
Macau is about 33 square kilometres and among the densest places on earth. Buses are cheap and frequent, taxis are plentiful, the free inter-resort shuttles form a useful de facto network, and residential parking is expensive and scarce. Many long-term residents never own a car and do not miss it.

Things most newcomers don’t know

The no-test exchange requires a Macao Resident Identity Card, which most foreign workers do not have.

The government's own service description for direct issue without test lists a valid Macao SAR Resident Identity Card among the requirements. Non-resident workers hold a blue card, not a BIR — so this route is not open to them on its face, and that is one more concrete consequence of the work-permit-versus-residence split. If you are on a blue card and need to drive, confirm your position with DSAT directly rather than assuming.

Left-hand traffic, ten minutes from right-hand traffic.

Macau and Hong Kong drive on the left; mainland China drives on the right. The Hong Kong–Zhuhai–Macau bridge and the Gongbei border crossing put those two systems within minutes of each other, and the switch is handled by dedicated interchanges. For a pedestrian the practical consequence is simpler and more urgent: look right first, every time.

Parking, not the licence, is what makes car ownership hard.

With roughly 690,000 people in 33 square kilometres, residential parking spaces are scarce and expensive, and many older peninsula buildings have none at all. Before buying a car, price a monthly space in your actual building. Plenty of residents conclude that taxis and the resort shuttles cover their needs for a fraction of the cost.

The free resort shuttles are a genuine transport network.

Every major integrated resort runs free buses between the ferry terminals, the border checkpoints, the airport and its own property. Together they cover most of the routes a resident actually needs, they run frequently, and nobody checks whether you are a guest. Learning the shuttle map is the single best transport hack in Macau.

Common mistakes to avoid

Assuming the no-test exchange is open to you when you hold a blue card rather than a resident ID
Arriving at DSAT without the supplementary documents your issuing jurisdiction requires
Trying to exchange a licence category you originally obtained without an examination
Buying a car before checking whether your building has a space you can actually rent
Stepping off the kerb looking left, as you would in mainland China
