Work out what covers you, and do not assume it is the state
Kyrgyzstan's Mandatory Health Insurance Fund is contribution-based and follows contribution history. On a local payroll you are normally inside it; on Digital Nomad status, a student visa, an e-visa or a visa-free stay you are not. Private cover is essential, and in Osh the argument for a policy with strong evacuation and repatriation limits is stronger than anywhere else in the country.

Find a private clinic, and know where the referral chain goes
Osh's private clinic sector is small but real, consultations are inexpensive, and the medical university means there are plenty of doctors. Understand the chain before you need it: routine and much acute care is handled locally; complex surgery, oncology and serious trauma route to Bishkek, 670 km and two passes away, or out of the country entirely. That is a decision made in an hour, not a week, and it is what your evacuation cover exists for.

Take the winter air seriously here too — it is a valley, not a bowl, but the smoke is the same
Osh sits at the mouth of the Ak-Buura valley where it opens into the Fergana. Winters are milder than Bishkek's, but household coal and wood heating is more widespread in the outer microdistricts and the private-sector housing, and still air over the valley concentrates it. There is far less monitoring here than in the capital, which means less data rather than less smoke. A HEPA purifier for the bedroom is the same cheap insurance it is in Bishkek.

Know the pharmacy system and the emergency numbers
Pharmacies (аптека) are dense and prescribing is looser than in Western Europe; pharmacists dispense and advise directly. Carry the ACTIVE INGREDIENT name of anything you take regularly. Ambulance is 103, police 102, fire and rescue 101, gas emergency 104. If you are going up into the Alay valley or toward Peak Lenin, put the national 24-hour mountain rescue line — +996 312 651 404 — in your phone before you leave, and understand that response times at that distance are measured in many hours.

Things most newcomers don’t know

Evacuation cover is the single most important line in an Osh insurance policy, and it is the line people economise on.

Osh is 670 km over two high passes from the country's best hospitals and considerably further from a major international medical centre. The referral chain out of here is real and it is expensive. A policy with a nominal evacuation limit is not cover in a landlocked mountain region — read the number, not the marketing.

The medical university gives Osh more doctors per head than its economy would predict, and that cuts both ways.

Osh State University runs an International Medicine department and the city has a large teaching-hospital layer, so access to a consultation is easy and cheap. But teaching capacity is not the same as tertiary capacity, and a city can have plenty of clinicians while still referring anything genuinely complex north or abroad. Judge the facility, not the headcount.

Air quality here is under-monitored rather than good, and the distinction matters.

Bishkek's winter smog is heavily instrumented and widely reported. Osh's is not, which produces the misleading impression that it does not have one. The physical drivers — coal and wood heating in outer housing, still winter air over a valley floor — are present. Treat the absence of published data as a gap in the record rather than as reassurance.

A travel policy bought for a visa checklist typically runs 90 days, and a medical degree runs six years.

This catches the city's largest foreign community every year. International students arrive on a 90-day student visa with a matching 90-day policy and then live here for years. Match the policy to the actual stay, check it covers the altitude and activities you will really do, and re-read the exclusions before the first trek.
