Before you start
- Employment with a Saudi entity, which carries the duty to insure you
- Passport and visa details for you and every dependant
- An Iqama application in progress, or an existing Iqama for renewal
- Absher plus Sehhaty and Tawakkalna for health identity and records
Step-by-step
- 1
Establish which scheme actually covers you
Most people hold a policy from a CHI-licensed insurer — Bupa Arabia, Tawuniya, MedGulf — meeting at least the Essential Benefit Package. If you are a contractor placed on a client's site, you are on the contractor's policy, not the client's, and the two can differ enormously. Site medical facilities are not the same thing as your insurance network.

Get the class and network list in writing at onboarding
Ask which tier you are on and which hospitals it admits you to. The cheapest compliant tier restricts your choices and can cap maternity, dental and pre-existing conditions. Onboarding is when this is negotiable; afterwards it is not.

Check dependants are on your class, not the floor
Cover for spouse and children on your Iqama is also the employer's legal obligation, but a company can lawfully buy you a good plan and them a minimum one. Verify the tier per person before family arrive, because a lower-tier dependant plan can lock them out of the hospital you use.

Confirm the policy is live on the CHI register
Enter your Iqama number on the CHI beneficiary service and check your health profile in Sehhaty. Jawazat and HRSD read this register automatically when issuing or renewing an Iqama, and nothing warns you if the policy is missing until the renewal quietly fails.

Pick your emergency hospital and save the route
Almana General Hospitals, Dr Sulaiman Al Habib and several others sit within Khobar itself, with more in Dhahran. Choose the nearest one on your network, drive the route once in daylight, and save it. Street addressing is weak here and a 3am search is not the time to work it out.

Know the numbers and how to describe your location
Call 911, live in the Eastern Province since 2022, or 997 for the Red Crescent directly. Give district, nearest major road and a landmark rather than a street name. If you are on the causeway or its approach, 996 reaches the highway patrol.

Things most newcomers don’t know
Bahrain is forty minutes away and is not a healthcare fallback.
It is a foreign country. Your Saudi cooperative policy will not generally pay a Bahraini provider, crossing requires a valid exit and re-entry permit, and the causeway can queue for hours on exactly the weekends when you would want to go. For anything urgent the Khobar and Dhahran private hospitals are both closer and covered. The causeway is a leisure route; treating it as a medical one is how people end up paying out of pocket in another currency.

The policy is a gate on your residency, and the failure is silent.
Jawazat and HRSD verify the CHI register electronically before issuing or renewing an Iqama. A gap left when an employer switched insurers produces no warning — it produces a renewal that will not go through, which then freezes your bank account, invalidates your driving licence and puts your family's status in question. Check the register yourself before each renewal window rather than trusting that someone else did.

Two people on the same site can be on completely different cover.
A direct hire and a contractor placement working side by side in Dhahran often hold very different policies, and access to a client's site clinic does not extend to your family or to your evenings and weekends. Establish which policy covers you off-site, because that is where you and your family will actually use it — the site clinic is for the site.

Gulf humidity injures people at lower temperatures than the thermometer suggests.
Coastal nights stay warm and sweat evaporates poorly, so the body gets little overnight recovery from heat load and heat exhaustion arrives sooner than in Riyadh's dry heat at the same reading. The Kingdom applies midday outdoor-work restrictions in the hottest months; if you supervise field or site teams, know the current dates and treat them as binding rather than advisory.

Common mistakes to avoid
- Accepting 'you're covered' from HR without the tier and the network list.
- Assuming a client site's medical facilities extend to you off-site or to your family.
- Letting dependants sit on a lower tier and finding they cannot use your hospital.
- Planning to use Bahraini healthcare on a Saudi cooperative policy.
- Changing employer under the mobility rules and leaving an insurance gap that blocks the Iqama transfer.
