You choose your primary care centre and list with it — a free choice within the region, done online with BankID or on paper. Doing this before you are ill means you have a named point of contact rather than starting from the emergency department.

Call 1177 before going anywhere
1177 is answered by registered nurses around the clock. They assess, advise, and direct you to the right level of care — self-care, your vårdcentral, an out-of-hours clinic or the emergency department. Turning up at an emergency department without doing this is the classic newcomer mistake and produces very long waits.

Keep every receipt for the high-cost ceiling
Once your patient fees in a twelve-month period reach the högkostnadsskydd ceiling, further care is free for the rest of that period. Prescriptions have a separate ceiling with its own sliding scale. Both are tracked digitally now, but check yours rather than assuming.

Adult dental care is subsidised through a separate national scheme with a modest annual allowance and a high-cost protection that only starts at a significant level of spending. Physiotherapy, psychology and specialist access run through referral or through the region's own choice systems.

Between arriving and being entered in the population register you are not yet in the system. EU/EEA arrivals use the EHIC; everyone else needs private cover for that window, which can be several weeks.

Things most newcomers don’t know
1177 is a triage system, not an information line, and using it changes what care you get.
Calling 1177 puts a registered nurse between you and the system, and they can book you directly into the right service. Skipping it and going to an emergency department means being triaged at the back of a queue behind genuine emergencies, sometimes for many hours. In Stockholm the difference between the two routes for the same complaint can be an afternoon and a whole day.

The care guarantee gives you a legal right to go elsewhere when the wait is too long.
Sweden's national care guarantee sets maximum waiting times for contact, assessment and treatment. If your region cannot meet them, it must offer you care with another provider — including in another region — at no extra cost to you. Regions do not volunteer this and the onus is on the patient to ask. It is the most valuable and least used right in the Swedish system.

Healthcare is the region's, not the state's, so moving between Swedish cities changes the details.
Region Stockholm sets its own patient fees, runs its own choice-of-provider system and organises out-of-hours care its own way. A colleague's advice from Gothenburg or Malmö about which fee applies or how to reach a specialist may simply be wrong here. The 1177 number and the high-cost ceiling are national; almost everything below them is regional.
